By signing it, or selection Start-button
below, you expressed consent to the processing of personal data and accept this
Agreement, incl. the Disclaimer.
The
client (data subject) hereby declares that he/she gives the User his/her
explicit and unqualified consent to process his/her personal data in that
includes email, phone number, date of birth, address, for the purpose of
identification and information support related to the KME-system Analysis and
compensatory correction of the current health status of the client during the
health care period.
The
client has the right to withdraw his consent at any time.
The
withdrawal of consent shall not affect the lawfulness of the processing
resulting from the consent prior to its withdrawal. At the end of the health
care period, the personal and health data of the individual client, at his
request, may will be discarded.
The use of the KME Anti-Age System and
the ERI-qlife app does not replace clinical, laboratory or other medical
examinations. The processes and conditions indicated in the analysis results
are not the basis for determining diseases or making a diagnosis.

Disclaimer statement for KME Anti-Age System use,
consultation, and evaluation:
1.
I hereby declare that in accordance with Article 13 of General Data Protection
Regulation 2016/679 of the EU on the protection of individuals regarding the
processing of personal data and on the free movement of such data (GDPR) I was
informed about the rights of the data subject when processing data in
accordance with the GDPR.
2.
I am aware that the CME Swiss AG, as a developer and manufacturer of
the KME-systems and ERI-qlife application, cannot become liable for usage of
the KME Anti-Age system by anyone, at any time.
3.
I am aware that consultation services provided by an KME user may not have
allopathic or medical genesis. The service of KME-system is only an alternative
or an additional service to the care and healing practices acknowledged by the
local administrations.
4.
I am also aware that these services are not recognized by the health insurance
companies.
5. I hereby declare that I have been
informed about the way and possibilities of the KME-system analysis. I am aware
that diseases will be neither diagnosed nor treated and that only my health
well-being, will be analyzed. I
declare that I understand the principle of using the system, and therefore I
agree to perform an analysis of the state of the body, including compensation
for the detected unbalanced processes.
6.
I am aware that the KME-user exclusively offers services originating from a
natural health system by using the KME-system for analysis and reduction for
influences are imbalanced processes, environmental state, and stress.
7.
I am aware of the fact, that the KME-analysis and correction do not replace the
classical medical examination, it does not exclude the visit of the doctor and
the treatment with classical medicine.
8. Checking the possibility of an
additional clause of the agreement.
